(1) A person whose name is removed under section 17(1) may, within 2 years from the expiry of the registration concerned, apply to the Building Authority for the restoration of the personˇ¦s name to the register.
(2) The application mustˇX

(a) be in the specified form; and
(b) be accompanied by the prescribed fee.

(a) require the applicant to provide any information and documentary proof that the Authority considers necessary for determining the application; and
(b) refuse the application if the information or proof is not provided.
