(1) An application for registration as a physiotherapist under section 13 of the Ordinance or for provisional registration as a physiotherapist under section 15 of the Ordinance, as the case may be, shall be in accordance with Form 1 in Schedule 2.
(2) A form of application for registration or provisional registration shall be signed in the presence of a barrister, commissioner for oaths, Part Ia physiotherapist, registered medical practitioner or solicitor and shall be delivered to the Secretary together with 4 photographs of the applicant of such size as shall be specified by the Secretary. (3) The Secretary shall refer the application of a person claiming to be qualified to be registered-

(a) under section 12(1)(a) or 15 of the Ordinance to the Board; and
(b) under section 12(1)(b) or (c) of the Ordinance to the Council,
