(1) Every supervisor on duty in a quarry who makes an inspection under regulation 38(1) or (2) shall forthwith enter in the Equipment Register a full report of the result of his inspection including details of any prohibition made by him under regulation 39(1). (See First Schedule, Form 2)
(2) Any supervisor on duty who contravenes paragraph (1) shall be guilty of an offence.
